Starting situation

The clinic ran on a legacy booking system that did not find calendar gaps for clients, did not prevent appointment clashes, did not assign appointments to clinicians or locations and did not check clinician capacity before booking. Patients clicked through packed day-by-day calendars on a slow page with no view of the soonest available slot. Clinicians arrived to find themselves triple-booked with no time to review notes before appointments. Office admins held the entire business process in their heads and managed it by hand across therapy reports, lab tests, schedules, report preparation, team meetings, payments, refunds and cancellations. Because the system could not link to the user portal, health records or payment and invoicing, the same information was entered into several systems repeatedly.

What we advised

We treated this as a business analysis problem rather than a software problem. The core issue was flawed assumptions and insufficient analysis of actual needs, so despite the client's twelve years of industry experience we began with a thorough discovery phase that challenged the status quo.

What we built or changed

We ran an in-depth as-is and to-be analysis combined with data analysis to pinpoint every bottleneck, and interviewed patients, clinicians and administrators to understand the system from every perspective. Those interviews uncovered the business logic behind booking rules, practitioner roles, appointment priorities and the time required for pre- and post-appointment activities. That revealed the critical insight: the booking system was not a scheduling tool but the operational heart of the clinic, needing to standardize patient pathways, manage practitioner time and on-site capacity, and monitor income and billed clinician hours. We then automated every task that could be automated and integrated the data flow end to end.
